To see what BayCarePlus (HMO) Medicare plans are available in your area, please click on the county where you live. If you don’t live in one of these counties, you may not be eligible to join a BayCarePlus plan.
BayCare Health Plans is an HMO plan with a Medicare contract. Enrollment in BayCare Health Plans depends on contract renewal. All BayCare Health Plans include Part D drug coverage. To enroll, you must have both Medicare Parts A and B and reside in the Florida counties of Hillsborough, Pasco, Pinellas and Polk.
You must continue to pay your Medicare Part B premium. Enrollment may be limited to specific times of the year. This information is not a complete description of benefits. Members must use plan providers except in emergency or urgent care situations. If a member obtains routine care from an out-of-network provider without prior approval from BayCare Health Plans, neither Medicare nor BayCare Health Plans will be responsible for the costs.
